چکیده

http://twitter.com/hsru_aberdeen Ureteric colic is the pain felt when a stone passes down the ureter from the kidney to the bladder and is a frequent reason for people to seek emergency health care. Treatment with the muscle-relaxant drugs tamsulosin and nifedipine as medical expulsive therapy (MET) improves the likelihood of spontaneous stone passage and lessens the need for interventional procedures. However, there remains considerable uncertainty around the effectiveness of these drugs for routine use.

Use of drug therapy in the management of symptomatic ureteric stones in hospitalized adults (SUSPEND), a multicentre, placebo-controlled, randomized trial of a calcium-channel blocker (nifedipine) and an a-blocker (tamsulosin): study protocol for a randomized controlled trial

Background: Urinary stone disease is common, with an estimated prevalence among the general population of 2% to 3%. Ureteric stones can cause severe pain and have a significant impact on quality of life, accounting for over 15,000 hospital admissions in England annually. Uncomplicated cases of smaller stones in the lower ureter are traditionally treated expectantly. Those who fail standard care...

Should we SUSPEND MET? Not really

Medical expulsive therapy (MET) in ureteric stones has 2 goals: 1) accelerate stone passage and therefore avoid surgery and 2) reduce analgesic requirement. In this setting, a RCT [1] done prior to the publication of the SUSPEND trial [2] is published in this issue of the CEJU, comparing the efficacy of tadalafil against tamsulosin in MET. It is regrettable that a placebo arm was not included ...

The efficacy of tamsulosin vs. nifedipine for the medical expulsive therapy of distal ureteric stones: A randomised clinical trial

OBJECTIVES To assess and compare, in a randomised clinical trial, the efficacy of tamsulosin and nifedipine as medical expulsive therapy for distal ureterolithiasis. PATIENTS AND METHODS In all, 128 symptomatic patients with stones in the juxtavesical tract of the ureter were randomly divided into group 1 (64 patients) receiving oral nifedipine sustained-release 30 mg/day, and group 2 (64 pat...

Ureteric colic: evidence empowers responsible treatment

www.thelancet.com Vol 386 July 25, 2015 315 The urological community has for decades pursued studies of so-called off -label drugs—ie, pharmaceutical agents used for an unapproved or unintended indication—for treatment of urolithiasis. But perhaps urologists should question why the pharmaceutical industry has not expressed an interest in drug development for this indication. Most urologists mig...
